Older People SAP Assessment

Forming a key part of the NHS policy for older people, The Single Assessment Process, or SAP, aims to ensure that: older people receive appropriate, effective and timely responses to their health and social care needs and that professional resources are used effectively.

The Needs Assessment Form for Older People was developed by InterRAI as an off-the-shelf assessment tool, and is published by TSO in support of SAP. Its components map directly onto the four levels of assessment specified in the SAP: initial contact, overview, in-depth and comprehensive. These can stand-alone or make up part of the whole picture.
