Forming a key part of the NHS policy for older people, The Single Assessment Process, or SAP, aims to ensure that: older people receive appropriate, effective and timely responses to their health and social care needs and that professional resources are used effectively.

The Needs Assessment Form for Older People was developed by InterRAI as an off-the-shelf, assessment tool, and is published by TSO in support of SAP. Its components map directly onto the four levels of assessment specified in the SAP: initial contact, overview, in-depth and comprehensive.

These can stand-alone or make up part of the whole picture. Logical links and assessment protocols help the assessor make a sound decision for each client. TSO provides assessment tools for the contact, the overview and the comprehensive assessment.

The training pack includes the following:

SAP Ability Assessment for Older People - the MDS User Manual introduces the assessments available from TSO, published in support of the Single Assessment Process.

The MDS Contact Assessment includes the self-reliance algorithm which indicates who should receive a full Overview Assessment.

The MDS Overview Assessment contains triggers to assessment protocols that assist the professional in determining next steps for care planning, including consideration of where other assessment domains are relevant for identified needs, and which specialist professionals should be involved in further assessment.

The MDS-HC Comprehensive Assessment provides evidence to support determination of the Registered Nurse Contribution to Care (RNCC) for people admitted to nursing homes and NHS-funded continuing care. These levels can only be determined using specialist computer software or through Senectus.
